ADMISSION CREDENTIALS

Incoming Grade 7

  • SF 9/Form 138 (Grade 6 Report Card) with Learner’s Reference Number (LRN)
  • PSA Birth Certificate (1 copy – Original; 1 copy – Xerox)
  • 2 x 2 ID picture with white background ( 2 pcs)
  • Original Copy of Certificate of Good Moral Character
  • Photocopy of Income Tax Return of Parents
  • Certification of Honors Received (if applicable) 

Transfer-in Students

  • SF 9/Form 138 (Report Card) with Learner’s Reference Number (LRN)
  • PSA Birth Certificate (1 copy – Original; 1 copy – Xerox)
  • 2 x 2 ID picture with white background (2 pcs)
  • Original Copy of Certificate of Good Moral Character
  • Copy of ESC Certification (for ESC Grantees only)
  • Copy of Income Tax Return of Parents (students from public schools)

 

Foreign Students

  • 2×2 passport size ID picture
  • Photocopy of passport data page (page containing student’s full name, date and place of birth, passport number, and photo).
  • Special Study Permit (SSP)
  • Notarized Affidavit of Support with proof of adequate financial support
  • Certificate of Registration        

ADMISSION AND ENROLLMENT PROCEDURE

  1. Issuance of Forms
    Get and fill-out the forms issued from the Staff in-charge after presenting your credentials.
  2. Verification of Forms
    Submit filled-out forms and wait for the Staff in-charge for the verification.
  3. Signing of Forms
    Proceed to the Principal’s Office for the signing of forms.
  4. Registration and Admission
    Proceed to the Digital Library for the registration and admission of data needed in the ESMIS portal.
  5. Admission – SIS
    Submit all forms needed for final encoding of entries in the SIS portal at the Registrar’s Office.
  6. Payment
    Pay for an initial amount of P5,000.00 at the Cashier’s Office.
  7. Intake Interview
    Have the Intake Interview at the Guidance and Testing Office.
